Child Initial Consultation Form

Thank you for your interest in Naomi’s Garden Conductive Education. In order for us to have the best understanding of your child and how to meet their individual needs, please fill out this form with as much detail as possible. After you have returned the completed form to us, we will be in contact with you to arrange your child’s initial consultation. Please note, your initial consultation will be free of charge and will be followed by a full Conductive Education report.

Below are some activities, tasks and skills that will be included in your child’s at Naomi’s Garden. It is important for us that we get a greater understanding of your child and how your child may complete certain tasks in order for us to tailor our Initial Consultation to the needs of your child. There are boxes numbered 1-5, 1 being your child needs constant support in the area, 3 is needing some help and 5 being your child can complete the task independently. If there are some areas that are not applicable to your child due to being a young age, just mark them N/A.
